Here is the work I did for my knife sheath. Again, it's not screen accurate, but it works so I like. I took an old 9mm magazine pouch that is SIDE MOUNT (little harder to find, it sits left to right on the belt rather up and down). I seam ripped out the clips and the cover and the bottom lacing, making basically a rectangular cloth tube.

I cut/burned (it's a nylon fabric) that hole to allow for the sheath snap. More on that later.
I marked off center points on the rear to add snaps. These snaps will be added to the webbing on the armor.

I made slight cuts to open a hole, added the snap base, and trimmed to fit.

Then I just completed the snap.

I seam ripped a lot on the original sheath and basically removed the MOLLE portion and folded it double. I didn't take any pics of that, but it wasn't too hard to do really. I left it double since it added enough bulk to friction lock the two pieces together.
When that was done, I slowly forced the sheath into the green pouch.

It's in there solid and tight. Not coming out. Previously I have used velcro added to the inside to lock it in place, but this sheath didn't need it. The combination of the thickness of the sheath and the cutout for the snap keep it from moving at all when the knife is drawn.
Finished product:

I'm leaving the velcro straps on the back for two reasons: 1) it will help secure it to the shoulder webbing. 2) it will cover the snaps on the shoulder webbing and help prevent damage to the armor.
