This is a cut template.

You can also use it as reference for the backing plates, but you would need to scale them up a tad.

There's only a portion of the left side that is flush with the helmet.

The left side is from your master while the right side and the sight unit are build from original (and a few manufactured) parts.
The hose connectors on the right side are made from the grips of precision screwdrivers, the conical part is turned aluminium (I used my drill as a lathe) and the cage was made from laser cut sheet aluminium bend into shape. Couldn't find the correct mike boom, so I modified the parts from one of the standard mike booms to match those in Gio's pictures.

Hi Matt,

here's a quick and dirty schematic of how it was done:

I wrapped a strip of 1mm aluminium (light grey) around the two wire ends from the mike boom that come from the helmet. Next I placed the joint plates (mid grey) from the standard mike boom on the front and back of the aluminium strip and drilled out the hole for the mounting screw. A test fit showed that I didn't get enough pressure onto the ball (blue) and thus I sanded down one side of the aluminium strip to a wedge shape.
With the strip done I filled the holes and indents of the two joint plates and sanded them into shape. A quick shot of black paint later it looked like in this (sadly not too sharp) picture:
